I hereby acknowledge that I am volunteering my services to Chi-Care, a nonprofit organization, without any expectation of monetary compensation. I understand that my participation in volunteer activities with Chi-Care may involve certain risks and hazards, including but not limited to physical injury, property damage, or other dangers inherent to the activities involved.
In consideration of being permitted to participate in volunteer activities with Chi-Care, I voluntarily agree to assume all risks associated with such participation. I hereby release, waive, discharge, and covenant not to sue Chi-Care, its officers, directors, employees, agents, and representatives from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me while participating in volunteer activities with Chi-Care, whether caused by the negligence of Chi-Care or otherwise.
I understand that Chi-Care will make reasonable efforts to provide instructions and guidance on safety procedures and protocols relevant to the volunteer activities. However, I acknowledge that it is ultimately my responsibility to adhere to such instructions and to exercise caution and good judgment while volunteering.
I grant Chi-Care permission to use my name, likeness, voice, and statements in any media, including photographs, videos, and recordings, for promotional, educational, or other purposes related to the organization's mission, without compensation.
I certify that I am voluntarily participating in volunteer activities with Chi-Care and that I am under no obligation to do so. I further understand that this waiver and release of liability shall be binding upon me, my heirs, executors, administrators, legal representatives, and assigns.
